Medicaid Service Coordinator
Job Description
The Medicaid Service Coordinator, in conjunction with the person and his/her advocate, is responsible for coordinating and directing the person’s individualized service environment (ISE). The Medicaid Service Coordinator assists Devereux Community Residents to gain access to necessary services and supports appropriate to the needs of the individual and develops an Individual Service Environment and Individualized Service Plan (ISP). The Service Coordinator is an employee of the Devereux in New York; however, primary responsibility is to the individual receiving services and to that person, advocate/family. Must be able to visit a variety of service provider locations. Must possess a valid New York State Driver’s license.
Job Requirements
EDUCATION: Associate’s degree in a health or human service field, or an RN required. Bachelor?s Degree in a health or human services field preferred.
EXPERIENCE: One year experience working with people with a developmental disability, or one year of experience as a service coordinator with any population. This experience may be waived if the person possesses a master?s degree in a health or human service field.
